Caremark has partnered with Subimo to bring employees the Coverage Advisor, a decision support tool. The Coverage Advisor is an easy-to-use online tool that helps you make better decisions for better health. While the basic plan design information for each medical option is clearly summarized in the Benefits Guide, there is so much more to consider when choosing a medical plan.

Please note, this tool is not available to PharmaCare employees at this time.

For instance:

•Are you or your family members healthy or living with chronic conditions that require regular medical services?

•Are you planning specific treatments or do you have a surgery planned for 2008?

•Is preventative care important to you?

•Are you planning on delivering a baby in 2008?

•Is the physician office co-pay cost a major factor in your decision, or is co-insurance more important to you?

•Did you meet your deductible this year?

Use the Coverage Advisor tool to do the following:

•Create customized family health profiles for all members of your family

•Estimate the medical services you will need in the year ahead

•Estimate your out-of-pocket expenses

•Choose the medical plan that is right for you
