Adoption Reimbursement Program

At Caremark, we recognize that adopting a child is an important family event! Through the Adoption Reimbursement Program, eligible employees will be reimbursed $5,000 per adoption to cover expenses that are reasonable and necessary for the legal adoption of a child. Some of these expenses include:

Court and legal fees
Foreign adoption expenses
Maternity fees (i.e., medical expenses for the child’s birth mother)
Pre-adoption medical examination and treatment
Public or private agency fees
Temporary foster care expenses


Adoption Reimbursement Eligibility

This program is available to all regular full-time and part-time employees who have completed six months and 1000 hours of employment with Caremark. Eligible adoptions are limited to children under the age of 19 years.



How it Works

Once you have incurred and paid adoption expenses, you may apply for reimbursement by submitting an Adoption Reimbursement form and your adoption receipts to the Caremark Benefits Center. Adoption Reimbursement forms may be obtained by clicking here, or by contacting the Caremark Benefits Center at 1-888-769-1717. Once your Adoption Reimbursement form has been approved by Benefits, you will receive your reimbursement in your payroll check. Current IRS regulations consider adoption reimbursement as taxable income, and therefore, reimbursements will have taxes withheld.
